Radiology uses the same order-at-payment, worklist and reporting patterns as the lab, and adds full imaging integration: DICOM modality worklist, Orthanc PACS, an embedded OHIF viewer and structured reporting.

Study lifecycle & worklist

order to acquisition

  • Lifecycle: scheduled → checked-in → acquired → ready-to-read → preliminary → finalised → delivered, plus reject/retake branches, with every transition recorded.
  • Technologist worklist: check-in, acquisition with radiation dose and contrast recorded, reject and retake.
  • Order at payment, using the same pattern as the lab, so every study reaches the patient's bill.

DICOM integration

  • DICOM modality worklist served to scanners through Orthanc.
  • Orthanc PACS for browsing studies, series and instances, with an embedded OHIF viewer.
  • Four-tier matching: arriving studies are matched to their orders by a webhook secured with a shared secret.
  • Attach queue: studies that cannot be matched wait there for manual resolution instead of being lost.

Structured reporting

  • Draft → preliminary → final with single-step or two-step approval. The author and the approver must be different people.
  • Immutable addenda and amendments; the original report is never edited.
  • Key images shown to the referring doctor, and critical findings with acknowledgement.
  • QR-verifiable printed reports.
  • 633 templates across 7 modalities, with RSNA RadReport import/export.

Operations & analytics

  • Equipment fleet with location history.
  • Turnaround time from order to image to report.
  • Workload by modality and radiologist productivity.
  • Finance exports into the radiology unit's own books (see Accounting).

03 dicom modality simulator

To test the imaging path without a scanner, I wrote a DICOM modality simulator in Go using only the standard library, including its own DICOM tag patcher.
